Five-time Grammy award-winning singer Sandi Patty announced her final tour Monday on NBC's "Today Show" while speaking with her friend, and co-host of the show, Kathie Lee Gifford.

Patty plans to bid her fans a final farewell on the upcoming "Forever Grateful" world tour, which is scheduled to begin in February 2016 and finish at the end of next year.

The singer is partnering with charity and child sponsoring organization World Vision for the tour. Patty has worked with World Vision for the past 30 years, performing at various events hosted by the charity.

The "Forever Grateful" tour will feature performances from the Veritas as well as members of her own family.

"What a privilege it has been to share my story and faith through song for so many years," said Patty in a statement shared with The Christian Post. "My heart is full of gratitude for so many who have been along this journey with me."

"I could never have imagined my career taking me to so many amazing places and meeting so many people who have changed my life forever. ... My desire is to use this next season and the 'Forever Grateful' tour as an opportunity to say 'thank you' to as many people as possible!"

Along with the tour, Patty will release a new album titled Forever Grateful in the beginning of next year. It will feature new music written by some of the biggest names in Christian music, including Michael W. Smith and Dove award winner Nichole Nordeman.

Patty rose to fame after performing a beautiful rendition of "The Star Spangled Banner" during the rededication of the Statue of Liberty in 1986. The rendition was so beloved, she was asked to perform it again at the Indianapolis 500, the Dedication of Camp David Chapel and ABC's "Fourth of July Special."

Other notable achievements by the singer include performing at three separate presidential inaugurations, headlining sold-out performances of the musical "Hello Dolly!" and winning over 40 Dove awards.

Patty has also sold over 12 million records during the span of her career.

Her final tour is scheduled to travel to more than 90 cities worldwide and includes 20 signature holiday tour dates. Tickets for the farewell tour go on sale in November.
